Nationally Recognized Expertise
Care Support of America (CSA) is a pioneer in providing Advanced Illness Coordinated CareTM services. Founded and lead by Dan Tobin, MD, a nationally recognized leader in care coordination and healthcare counseling, who has trained and supervised hundreds of nurses in public and private settings, including all the CSA on-staff nurses. Our registered nurses average over 10 years each of geriatric nursing care experience.
The Care Support of America program is the result of millions of dollars of research, conducted over 8 years, by the leadership of CSA.
When compared with usual health care delivery, research findings indicate:
- Decreased hospital admissions and more time at home 2, 3
- Improved patient and caregiver satisfaction with healthcare delivery 1, 2
- Enhanced frequency and timeliness in formation of advance directives1,2, 3
- Improved communication with professional health providers 1, 3
