Personal Action Plan for Aging Parents

Our Family Care Managers are registered nurses who average over 10 years each of elder care experience. Working directly with you to understand your situation and your aging parents’ healthcare needs, your Family Care Manager will develop a plan that enables your family to receive the support they need—whether it’s arranging home care services for elderly parents or simply listening to the stresses of being a primary caregiver.

Care Support of America has shown that, in an average of three to four hours, we have helped families address a wide range of specific caregiving problems with great success.

Initial analysis, action plan, and care coordination fee:  $480 (3 hours)

Continuing care coordination fee:  $130 per hour


Your Family Care Manager will:

  • Develop a caregiving problem list and prepare an individualized action plan to coordinate solutions for and with you
  • Help in solving immediate problems or crises
  • Organize all aspects of returning home from a hospital
  • Arrange for a local nurse to visit your parents and help ensure their safety at home
  • Identify and coordinate local home care services such as private duty in-home aides, nurses and equipment
  • Schedule local nurse visits to your parents in the hospital, assisted living facility, or nursing home, or to accompany them to doctor visits
  • Assist with Medicare, long-term care insurance and paperwork
  • Help with scheduling and managing doctor visits, medical test appointments and medication management
  • Provide a customized family web portal to keep family members informed of progress
  • Arrange conference calls with your family to inform them on updates and progress
