Full and Part Time Registered Nurse / Case Manager (Telephonic Care Services)

CSA has several full and part time openings for experienced Registered Nurses in the field of Advanced Illness and preparation for end of life care.

The Registered Nurse will provide advanced illness and end of life preparation services. Using the Advanced Illness Coordinated Care Programs, the RN will counsel patients and their families through health education programs designed to improve quality and timely end of life care.

Responsibilities include assessing, planning and coordinating the delivery of services for patients who are facing advanced illness.

Candidates should be a graduate from an accredited School of Nursing and currently licensed as a Registered Nurse. Must have minimum of three years experience in the field of preparation for end of life care. Certified Home Health, Hospice, Behavioral Health and/or Geriatrics experience is preferred. BSN or CCM preferred.
