Care Support of America's Advanced Illness Coordinated Care service assists patients and their families coping with serious chronic illness. AICC has been proven to improve quality of life and reduce hospitalization with no increase in mortality.
What CSA Provides
An experienced Registered Nurse (RN) will provide the Member and average of 3 months of telephone based counseling and care coordination, as well as providing a community nurse to visit the patient's home to assess safety. CSA integrates with case managers and physicians and most importantly, family caregivers, to provide a proactive approach to Advanced Illness care.
How It Works
Three step approach:
Identification, Engagement, and Intervention
Members are identified for the program through predictive modeling using existing data as well as direct referral from case managers, physician offices and hospital discharge planners.
Engagement occurs when the Members and families are paired with their own experienced RN for individual assessment and care. The CSA nurses are specially trained and experienced in individualized Advanced Illness, strengths-based problem solving.
Intervention begins with an initial telephone assessment involving the Member and primary family caregiver followed by an in-home safety assessment by local nurses as needed.
The CSA nurse works with the Member and family caregivers to develop an individualized plan for care, including identification of problems and care solutions.
CSA is able to help the patient and family:
- clearly define, prioritize and address problems
- understand the illness
- cope with medical and non medical issues
- prepare helpful questions for doctor visits
- plan for future needs
AICC is a non-directive health counseling approach, focusing on
relationship based, psychodynamically- oriented nursing.
